The information will assist us in choosing a puppy that will compliment your family.

If you are sure you would like a Labradoodle, your non-refundable deductible booking fee of $500.00 should accompany this form. Puppies are pedigreed and tattooed for life long identification and Over the Moon Australian Labradoodles extends to you ongoing support and advice.
